Glasses Voucher Application

Soul Eminence Center partners with vision programs to help individuals and families access eyeglasses they otherwise couldn't afford. Complete the application below to get started.

Before you apply

•Applicants must have an eyeglasses prescription dated within the last 24 months, including their pupillary distance (PD) measurement.
•Applications are submitted online. It is optimal for social service agencies/health advocates to apply on a client's behalf.
•Each application has a non-refundable application fee for administrative costs.
•If the application is approved, a unique e-voucher number is emailed with a link to our eyeglass ordering site and instructions on how to order eyeglasses.
